Why Do I Need An Identity Declaration?

The Identity Declaration is an essential step in applying for Australian Citizenship. Quite simply, if you cannot prove your identity you will not be granted Australian Citizenship.

Do I Need To Use This Form?

Every individual applying online for citizenship or proof of citizenship must submit a separate Form 1195. So if you are applying with your family, each member must provide their own form.

You also need to use this form if you are making a paper application and have run out of space for listing your children. You must submit a separate Form 1195 for each extra child.

Who Can Complete Form 1195?

You need to fill out Questions 1 and 4 of the form if you’re applying for citizenship or proof of citizenship. Meanwhile, someone who can validate your identity must complete Questions 2 and 3. Specifically, this person must be an Australian citizen, have known you for at least 12 months, and currently work in one of the occupations or professions listed on pages 1 and 2 of the form. Furthermore, they must not be related to you and should be easily contactable by telephone during the Department’s working hours.

What Else Will I Need?

A colour passport-style photograph to be signed as a true likeness by each applicant.

  1. I’m applying a paper application just myself I’m wondering if I still need to fill up form 1195 or I’ll just answer no. 36 on the form declaring identity by someone who knows me for a year or so? Thanks so much

  2. I don’t have close friends that are in the required occupations, I am asking people to fill out their personal details for my daughter, with the way the world operates today, they are hesitant even after knowing them at their work place over 10 years

  3. My daughter is 4 months old. And the person that will validate it needs to be 1 year or more that she knows my daughter. What should she write then? Please let me I’ve been stuck with this form so that i can attach it to her application

    1. We are in the same situation. My son is 6 weeks old and the local pharmacist says under question 3 he would write my name not my son’s. As he has known me for an year and not my son.

  4. I find that requirements on form 1195 is really unfair for most immigrants. Finding an Australian citizens who is working in the list of professions and has know us for over a year to sign our identity declaration is really really hard.
    My GP that I have been to for the past 8 years said she can’t sign as she isn’t a citizen and also working for a large clinic for someone and that her employer doesn’t allow signing forms for clients.
    She is the one and only person that is working in listed professions and has known me for over a year and not personally related. What do I do now? Where to find someone to sign for me?

  5. I have two kids with my application, they are 8 and 4 years old. Should I fill two separate forms for them or how should I do it?
