Form 47SP is completed by the visa applicant — the person migrating. Form 40SP is completed by the sponsor — the Australian citizen, permanent resident or eligible New Zealand citizen supporting the application. They are lodged together.

Yes. Disclose them honestly. A concealed refusal that later surfaces can be treated as false or misleading information and is far more damaging than a declared one.
