Why Do I Need An Identity Declaration?
The Identity Declaration is an essential step in applying for Australian Citizenship. Quite simply, if you cannot prove your identity you will not be granted Australian Citizenship.
Do I Need To Use This Form?
Every individual applying online for citizenship or proof of citizenship must submit a separate Form 1195. So if you are applying with your family, each member must provide their own form.
You also need to use this form if you are making a paper application and have run out of space for listing your children. You must submit a separate Form 1195 for each extra child.
Who Can Complete Form 1195?
Questions 1 and 4 of the form must be filled in by the applicant, i.e. the person applying for citizenship or proof of citizenship. Questions 2 and 3 must be completed by someone who can validate your identity. This person must be an Australian Citizen, have known you for at least 12 months and be currently working in one of the occupations or professions listed on pages 1 and 2 of the form. They also must not be related to you and be easily contactable by telephone during the Department’s working hours.
What Else Will I Need?
A colour passport-style photograph to be signed as a true likeness by each applicant.
